In consideration of my participating in the Flag Football Skills Camp (the “Program”), I hereby release the Winnipeg Football Club, the Winnipeg Blue Bombers, each of their affiliates, subsidiaries and related group of entities and member clubs, and each of their respective employees, officers, directors, sub-contractors, coaches, sponsors, representatives, and agents (collectively, the “Releasees”) from or for, as applicable, all present and future claims that may be made by the me, my family, estate, heirs or assigns for property damage, personal injury or wrongful death arising as a result of my participation in the Program, wherever, whenever, or however the same may occur. I understand and agree that the Releasees are not responsible for any injury or property damage arising out of the Program, even if caused by their ordinary negligence.
I understand that participation in the Program involves certain risks, including, but not limited to, serious injury, death or permanent disability. I am voluntarily participating in the Program with knowledge of the dangers involved and agree to accept all risks of such participation. I certify that I am in excellent physical health and may participate in strenuous and hazardous physical activities, including the football to be played in the Program. I also agree to indemnify and hold harmless Releasees for all claims arising out of my participation in the Program and all related activities. Emergency Medical Treatment I give consent to receive medical and dental treatment and/or first aid, including authorizing any medical treatment facility/hospital to administer emergency treatment for any injury, accident or illness during my participation in any activity of the Program. I acknowledge responsibility for reasonable charges in connection with the care and treatment rendered.
I understand that Program may be photographed, videotaped or otherwise recorded and that descriptions or accounts of the Program may be written or otherwise documented. I hereby grant permission to the Releasees to take photographs, videos or other recordings (collectively, the “Photographs and Videos”) of my at the Program. I agree to let the Releasees use my name, Photographs and Videos and/or likeness free of charge in any manner and in any media (now known or hereafter developed) worldwide, including but not limited to on television, video, the World Wide Web and Internet, without notice, review or approval, for any purpose and without any compensation, except where prohibited by law. I agree to waive any moral rights I may now or in the future have in the Photographs and Videos.
I hereby release, indemnify and hold harmless the Releasees from and against any claims, damages or liability arising from or related to the use of the Photographs and Videos and any related materials thereto made by me, my family, estate, heirs or assigns now or in the future. BY INDICATING MY ACCEPTANCE OF THIS RELEASE AND CONSENT, I AM AFFIRMING THAT I HAVE READ AND UNDERSTAND THIS RELEASE AND CONSENT AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE. I ACKNOWLEDGE THAT I AM SIGNING THE RELEASE AND CONSENT FREELY AND VOLUNTARILY, AND INTEND BY MY ACCEPTANCE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
* If the above individual is less than 18 years old, his or her parent or legal guardian consents to the preceding and indemnifies the Releasees for any loss it may suffer as noted in the above provisions. I hereby agree to all the information as detailed in the PARTICIPANT RELEASE, EMERGENCY TREATMENT & PHOTOGRAPH AND VIDEO CONSENT.
